Reclaiming a smile
Dr Chad Kasperowski runs his clinic, Champions for Oral Health, with the goal to treat patients holistically so that they can live complete and fulfilling lives. In this particular case, Chad treated a new patient whose chief complaint was jaw pain and upper arch veneers done by another dentist which kept falling off. In addition to these primary concerns, the patient was unhappy with the esthetics of her new smile as she felt the restorations were too bright and looked ‘short’.
The dentist who had done the previous work had also placed her in a lower mouthguard to wear full time. “Unfortunately, multiple restorations turned out to be luted incorrectly (many were zirconia based) and preps were dramatically over-prepared,” Chad explains.
Adding to the complexity of the case were two other factors: the patient is a former gymnast with a long history of neck injuries and subsequent TMJ and bruxing issues. “Capturing a stable bite and opening VDO to amend for long term occlusal wear was a challenge as well as finishing the case with less overbite,” he adds. Secondly, shortened roots of upper anterior teeth limited the amount of crown lengthening that could be accomplished.
In collaboration with the interdisciplinary specialists at the DSD Planning Center, the treatment plan was created to address the functional and esthetic challenges of this case with a natural-looking end result.

DSD Direct Lower Arch using the printed guides which Chad printed in-house. “We chose this as there were financial concerns on the patient’s part, but also because we wanted to really take our time ‘locking in’ a stable bite for this patient and THEN convert over time to porcelain restorations on the lower,” he elaborates.
All treatment was completed in one day using the guided technology.

Delivering Predictability and Patient Satisfaction
Chad explains that the fitting of the final restorations in the upper arch, milled from Vita Suprinity Lithium Silicate, went very smoothly.
“First of all, the restorations looked amazing in their vitality, shape and appearance. They fit like a glove with minimal adjustments,” he says.
“The margins were terrific and the gingival emergence and line angles employed by the DSD Lab is a step above… they truly understand how to use these optimally to help with the final result. Additionally, this case demanded ideal occlusal balance and once again, DSD delivered,” he adds.
Most importantly, the patient was very happy with the outcome from an esthetic and functional perspective. “The patient was elated with her new smile, but even more about the feel of her new bite.”
