In this article, we will show the results of two recent cases completed by Dr Charles Regalado, DSD Clinic owner from Spokane, Washington in collaboration with the DSD Planning Center and DSD Lab – both of which required natural-looking restorations in the esthetic zone.
Solving a complex implant case
The patient in the first case is a dentist himself who had missing upper lateral incisors restored with metal-backed Maryland Bridges. These were removed by a surgeon and the patient had implants placed in the lateral locations. He wanted a more natural-looking smile with improved color balance – and no more bridges.
“This was a very challenging case, since we had two implant crowns and normally it's difficult to match the final shade with the esthetic between crowns over implants and over teeth preparations," explains DSD Lab Clinical Mentor, Ângelo Raphael Coelho.
Other complexities about this case included:
The roots of central incisors and canines converged toward the missing lateral incisor spaces
The metal was bonded very securely
Once the veneers were removed on the centrals, the preps on the teeth were surprisingly dark

This case passed the ultimate test
As practitioners, it goes without saying that high-quality, ethical treatment - which puts the patient’s needs first - should always be our goal. A commonly referenced idea that ‘measures’ whether this is true of a procedure is the ‘daughter test’, in which a clinician asks themself a question: ‘Would I carry this out on my own daughter?’.
And for this case, it could be said that the treatment passed this test: the patient was thrilled with the results and recommended Dr Regalado's work to his daughter, who decided to also undergo treatment.
Similarly, this second case involved missing laterals with a ridge deficiency. “Charles did a great job planning to be as minimally invasive as possible by doing two Maryland bridges and conservative veneers on centrals and canines,” explains Christian Coachman.
“The results show the proper understanding of morphology and individualization by our lab team to create the sensation that laterals don’t look like poetics but yet, like teeth coming out of the gum and separated from the adjacent teeth. As always, understanding substrate color and material selection and restoration thickness, to achieve the final desired color.”
“Another detail about this case is that we applied a super-thin layer of ceramic inside the wings of the zirconia structure so that it could be adhesively cemented. We reduced and layered keeping the fitting,” adds Ângelo Raphael Coelho.
